The Clinical Care Coordination and Discharge Planning team works with patients throughout their UPMC treatment journey – from day one of admission to post-discharge – ensuring that patients are prepared for a successful discharge and set up to achieve continued improvement following inpatient care.
Ideal for social workers and registered nurses interested in care management, case management, or care coordination, you’ll partner with a multi-disciplinary team to support patients and their families by facilitating effective inpatient discharges and coordinating comprehensive post-discharge care. Not a nurse or social worker? This career track still has a place for you.

1 – Discharge Plan Coordinator role requires High School Diploma/GED.
2 – Discharge Plan Associate role requires Diploma or Associates Degree and RN License for Nursing track. Non-Nursing track requires a bachelor’s degree in social work or another health or human services field that promotes the physical, psychosocial, and/or vocational well-being of those being served.
